Rule 24.29.2607 - CERTIFICATION PROCESS

(1) Application for certification requires the following documentation:

(a) a completed SIF application form signed by the applicant; and

(b) medical evidence of permanent impairment signed by a physician qualified to complete impairment ratings.

(2) When the department approves an application, the department shall notify the applicant and referring agent, if applicable, and provide the applicant with a letter of SIF certification.

(3) The department shall retain an incomplete application for a period of one year. When an application remains incomplete one year after submission, the department shall deny the application.

(4) When the department denies an application, the department shall issue an order informing the applicant and referring agent, if applicable.
