(1) Fees are payable to the board.
(2) Fees are:
(a) limited license (speech-language
pathologist):

(iii) placement of license on

(iv) annual
renewal of inactive license 40
(c) audiologist:
(i) application 150

(d) speech-language pathologist and
audiologist (dual license):
(i) initial
