Rule 24.180.704 - APPLICATION FOR MEDICAL GAS PIPING ENDORSEMENT

(1) Any person required to obtain a medical gas endorsement shall make application to the board on the form prescribed by the board.
(2) Application for endorsement shall include:
(a) a completed
and signed application;
(b)
documentation that provides proof the applicant has successfully completed an
approved training program which meets the criteria of ASSE series 6000
professional qualification standards and a third-party testing source
acceptable to the board, and has obtained certification in the installation of
medical gas systems, based on NFPA 99C and Section IX of the ASME Welding and
Brazing Codes; and
(c) the
nonrefundable application fee.
(3) The board will review applications for endorsement on a case-by-case basis and issue endorsements to those applicants meeting the requirements imposed by 37-69-401, MCA.
(4) The board may, at its discretion, require additional documentation from an applicant to verify compliance with the requirements imposed by 37-69-401, MCA.
