(1) An application for a special recreation permit must include:
(a) a completed special recreation
application and permit form; and
(b) other relevant information in sufficient
detail to allow the department to evaluate the nature and impact of the
proposed activity, including measures the applicant will use to mitigate
adverse impacts.
