Section 24-8-4-1 - Phases of the Application Process

A. Applicant Phase
(1) Applicant submits to the Division a
complete application for admission to the Mental Health/Mental Retardation
Administrator Licensure program.
(2) Application materials are reviewed by the
Division for completeness.
(3)
Complete applications are forwarded by the Division to the Review Board for
review.
(4) Applications
recommended for approval by the Review Board go to the Executive Director for
review and approval/ disapproval.
(5) Upon approval of the Executive Director,
the Applicant is notified in writing of his/her admission to the program and
his/her new designation as a program Participant.
B. Participant Phase
(1) After admission to the program, the
Participant completes all Licensure Requirements. See Chapter 3, Section 5, pp.
9-13.
(2) Progress reports are
submitted to the Division by the Participant in accordance with the time frame
established by the Division. They are reviewed by the Division for
completeness. See Chapter 3, Section 5, C. and D., pp. 11-12 for more
information on progress report requirements.
(3) Upon successful completion of all
Independent Study and Training requirements, the Participant's complete file
(including material submitted with original application, progress reports and
related documentation) is submitted to the Review Board for review and a
recommendation of approval/ disapproval.
(4) Participant files recommended for
approval go to the Executive Director for review and approval/
disapproval.
(5) Upon approval of
the Executive Director, the Participant is notified in writing of his/her
successful completion of the program and his/her new designation as a Licensee.
Each such Licensee will be awarded a certificate attesting to the professional
title of Licensed Mental Health/Mental Retardation Administrator
(LMH/MRA).
