Rule 24-3-13.8 - Verification of Independent Study and Training Requirement - (LA)

A. Independent Study and Training Requirement Documentation:
1. If the LA applicant
completed Independent Study and Training Requirement Option One:
a. Copy of CPM Certificate and
b. Copy of Focus Certificate.
2. If the LA applicant completed
Independent Study and Training Requirement Option Two:
a. Copy of CPM Certificate
b.
The Division maintains a copy of
LA examination completion records; the applicant does not have to submit a copy
of this information.
B. Submitting copies of the CPM Certificate or the Focus Certificate (or other official proof of completion as accepted by the Division and/or the Review Board) is the applicant's responsibility; these certificates must be official copies issued by the sponsoring agency, as determined by the Division and/or by the DMH Human Resources office.
