A. The Division of Medicaid reimburses for:
1. One (1) retrospective review and
interpretation of blood glucose values per month, and
2. A one (1) time device hook-up which
includes beneficiary education.
B. The Division of Medicaid does not reimburse for a separate Evaluation and Management (E&M) visit unless a separately identifiable service is performed.
