Expiration of a License or a Permit and Conditions for Renewal
Section 2-1-3-11-01-108

Current through March 31, 2025

All Operator Licenses and Permits shall expire thirty-six months from the date of issuance. To renew such License or Permit, the holder shall submit a request for renewal on a form prescribed by the Bureau. In conjunction with the renewal form, the operator must demonstrate that he or she has, within 12 months preceding the expiration date, (1) attended a training course approved by the Bureau or (2) retaken and passed the appropriate examinations administered by the Bureau. It is the responsibility of the licensee or Permit holder to know when his/her Operator License or Permit expires and to get the Operator License or Permit renewed prior to the expiration date.
