A certificate for every death which occurs in this state shall be filed with the Office of Vital Records of the Department of Health within five days after such death occurs and shall be registered if it has been completed and filed in accordance with this rule. If unknown, the place of death is assumed to be that where the body is found. If the date of death is unknown, it shall be determined by approximation.
