Delayed registration when the applicant is six (6) years of age or over For an individual six (6) years of age or over, the delayed certificate of birth shall be signed and sworn to before a notary public by two (2) individuals, selected in the following order of priority, who are competent to sign and swear to the accuracy of the facts stated therein:

1. The person whose birth is to be registered, if that person is eighteen (18) years of age or older, or

2. One or both of the parents of the applicant, or

3. The legal guardian of the applicant, or

4. The next of kin of the applicant, or

5. Any adult having personal knowledge of the facts of birth.
