Upon receipt of the service fee and the acceptance of the signed delayed certificate form, the State Registrar, or authorized representative, shall abstract onto the delayed certificate a description of each piece of documentary evidence submitted to support the facts shown on the certificate. This delayed certificate shall then be used to register the birth and the applicant shall be provided with one certified copy of the certificate.
