One or more pieces of documentary evidence shall be provided which corroborate the facts set forth on the delayed certificate. Each document must include information as to the type of document and the date it was originally created or filed. The document(s) must establish:

1. The full name of the child at the time of birth,

2. The date of birth and place of birth,

3. The full maiden name of the mother,

4. The full name of the father, if the father's name is to be listed on the certificate. The father's name may be listed on the certificate only in compliance with Rule 3.3.2 of this subpart.
