In addition, the applicant shall furnish the State Registrar at least two pieces of documentary evidence which corroborate the facts set forth on the delayed certificate. Each document must contain identification as to the type of document and a date of creation or filing which is five (5) years or more prior to the application. These documents must establish:

1. The full name of the applicant at the time of birth;

2. The date of birth and place of birth;

3. The full maiden name of the mother;

4. The full name of the father, compliance with Rule 3.3.2 in this subpart is required if the father's name is to be listed. If the mother is married, a copy of the marriage license must also be furnished.
