Any emergency medical services provider, as defined in § 43-15-207, who takes possession of certain abandoned infants under §43-15201, or whoever assumes custody of a live born infant of unknown parentage shall file a certificate of live birth within five days and such certificate shall contain the following:

1. Have "Foundling" plainly marked in the top margin of the certificate.

2. The required facts which may be determined by approximation. A list of the required facts may be obtained from the Bureau of Vital Records.

3. The signature of the custodian and the custodian's title, if any.

4. The place where the child was found or taken into possession shall be entered as the place of birth.
