When a birth occurs and the cord is cut in a hospital or enroute thereto, the hospital shall be recorded as the place of birth and the person in charge of the hospital or his designated representative shall obtain the personal data and prepare the certificate. The completed certificate shall be presented to the mother for her approval and signature before she leaves the hospital. The physician or nurse-midwife in attendance shall provide the medical information required by the certificate and certify to the facts of birth within 72 hours after the birth. If that person does not certify to the facts of birth within the required 72 hours, the certificate may be signed by a medical staff member or the person in charge of the hospital. The completed certificate shall be filed with the Office of Vital Records of the Department of Health within five days after the date of birth.
