Whenever a cathodic protection survey is conducted in the State of Mississippi, the appropriate form (s) prescribed by the MDEQ (Appendix K and/or L) must be utilized to document the survey. However, use of the prescribed form(s) is not intended to limit other kinds of documentation that may be desirable in order to complete the evaluation. For instance, it may be necessary to provide a written narrative describing various aspects of the evaluation or a repair/modification that are not captured by completion of the form(s) them selves.
