CASE MANAGEMENT SERVICES

Subpart 1. Required records; children.
A case manager providing case management services to children with severe emotional disturbance must keep the records required in Minnesota Statutes, section 245.4881, subdivision 3, paragraph (b).
Subp. 2. Monitoring and recording outcomes.
The case manager shall monitor and record the attainment of service outcomes to determine whether:
A. the client's level of functioning is being
maintained or has changed;
B. the
services are being coordinated in a manner designed to assure continuity of
services needed by the child and to support the outcomes identified in the
child's individual family community support plan; or
C. in the case of an adult, services are
being coordinated in a manner to assure continuity of services needed by the
adult and to support the outcomes identified in the adult's individual
community support plan;
D. the
child who is age 17 and who may be eligible for case management services to
persons with serious and persistent mental illness receives information
necessary to make the transition to case management services for persons with
serious and persistent mental illness; and
E. the child and the child's parent or legal
representative or the adult receive information about applicable provisions of
the Patients and Residents of Health Care Facilities Bill of Rights, appeals of
denials, terminations, reductions or suspension of services, the release of
information under the Government Data Practices Act about services, and
authorization of services.
