Rule 5. An applicant shall ensure that its application for a special license is received in the offices of the Michigan liquor control commission, Lansing, Michigan, not less than 10 days before the date the license is desired. The president and secretary of the organization making application shall sign the application in the presence of 2 witnesses and the signatures shall be notarized. Further, an applicant shall submit, with the application, an affidavit showing the length of time the organization has been in existence.
