Section 3.05 - Responsibilities of Certified Community Health Workers to the Board

(1) Change of Personal Data. A Certified Community Health Worker shall provide written notice to the Board, in a form and manner prescribed by the Board, within 30 days of any changes to his or her:

(a) name;

(b) address of record; or

(c) social security number.

(2) Responses to Board Requests for Information. Except as otherwise provided by law, a Certified Community Health Worker shall provide timely responses to Board requests for information relating to:

(a) any application he or she has filed with the Board; or

(b) any investigation or formal complaint pending before the Board pursuant to 272 CMR 9.02: Investigation and Initiation of Formal Complaints.

(3) Reporting Actions by Other Licensing Authority. A Certified Community Health Worker shall provide written notice to the Board within 30 days of any action taken by a licensing or authorizing agency, within or outside the Commonwealth, against any certificate, registration or license which he or she may hold that authorizes healthcare related practice.
