Section 4.02 - Application for Examination

(1) Any person desiring to take an examination for certification as a public water system operator shall file an application on a form provided by the Board. The Board shall not accept any application that is incomplete or illegible.

(2) No examination application shall be considered complete unless it is accompanied by the required fees as determined by the Commissioner of Administration and Finance and all required supporting documents have been received by the Board or its designee. Incomplete applications shall be retained for 180 days.

(3) The application fee for examination as set by the Commissioner of Administration and Finance is non-refundable.

(4) An applicant may apply for, and take, the examination as many times as necessary and may take any grade examination.

(5) Members of the Board shall not permit their names to be used as a reference in connection with the filing of an application for examination as an operator of a public water system.

(6) Application for examination as an operator of a public water system shall be mailed or delivered to: Board of Certification of Operators of Drinking Water Supply Facilities, or to the Board's designee.
