(A) A registration fee shall accompany each application for initial or renewal registration. The fee shall be $750.00 for a single location, plus $300.00 for each additional separate location operated by the temporary nursing service agency.
(B) The fee for a transfer of ownership shall be $250.00 for a single location plus $100.00 for each additional separate location.
(C) The fee for a change of location shall be $100.00 for each location.
