A. Except as provided in Regulation .02B(2) of this chapter, the following individuals shall notify the State Superintendent of Schools in writing of charges against a certificate holder pursuant to Regulation .02 of this chapter:
B. The written report shall include all of the following:

The State Superintendent of Schools shall suspend or revoke a certificate pursuant to written notification by a local superintendent of schools, an administrator of a nonpublic school, or the Assistant Superintendent of Certification and Accreditation, of the charges against the certificate holder.
D. Before suspending or revoking a certificate, the State Superintendent of Schools shall:
E. The State Superintendent of Schools shall mail one copy of the charges and the procedures applicable to the suspension or revocation of a certificate by certified mail to the current or last known address of the certificate holder.
F. The certificate holder against whom charges have been filed may request a hearing in writing within 10 calendar days following receipt of the charges.
G. If the certificate holder does not request a hearing in writing on the charges within the 10-day period set forth in §D(2) of this regulation, the State Superintendent of Schools shall suspend or revoke the certificate holder's certificate.
