Section 10.21.16.05 - Program Service Plan (PSP)

On the forms approved by the Administration as part of the application required under Regulation .04C of this chapter or COMAR 10.21.04.05B, a program shall submit a PSP that includes a description of:

A. The range of services that the program will provide;

B. The populations to be served, including age groups and other relevant characteristics, and the number of individuals that the program expects to serve;

C. Program sites and hours of operation, and the method to ensure that services are accessible to the individuals served;

D. The goals, objectives, and expected outcomes of the program;

E. The composition of the program's governing body, board of directors, or advisory committee, whichever applies, as outlined in COMAR 10.21.17.05A;

F. The jurisdictions where services will be provided and the number of services to be provided in each jurisdiction; and

G. The jurisdiction where most of the program's services will be rendered.
