Chapter 162 - THE ADMINISTRATION OF THE INTERNATIONAL REGISTRATION PLAN
Section 250-162-6 - NAME AND ADDRESS CHANGES

1. Name Change

A. Any name change will require a written signed statement requesting the change and submission of proof of insurance and an updated MCS-150 reflecting the registrant's new name. New cab cards will be issued for all the registrant's vehicles at a fee of five dollars ($5.00) per cab card.

B. A registrant may change their name from an individual to a newly formed corporation provided the individual is the sole owner of the new corporation.

C. If the name change is from a corporation to an individual or another corporation, a sale is deemed to have occurred and a new registration is required on each vehicle. A registrant may submit documentation to show that a name change did not involve a sale.

2. Address Change

A. A registrant may request an address change by submitting a Schedule C and an updated MCS-150 to reflect the new address. A change in physical address also requires submitting three (3) forms of documentation listing the physical address.
