Chapter 5 - OCCUPATIONAL SAFETY AND HEALTH STANDARDS FOR PUBLIC SAFETY DIVING
Section 179-5-X - Recordkeeping

A. Each agency shall maintain written records of all operational and training dive activities. Records kept shall include:

1. Medical clearance for each diver.

2. Training records.

3. Equipment inspection/maintenance records.

4. Dive records. Dive records shall include, but not limited, to the following:
a. Type of Dive

b. Date

c. Name of Dive Supervisor

d. Name and Role of Dive Members

e. Location

f. Surface and Subsurface Environmental Conditions

g. Dive Profile

h. Unusual Incidents

B. Each agency shall retain all written records for active members. Records connected to non-active member shall be retained a minimum of five (5) years beyond the individuals last dive-related activity.
