Chapter 129 - PERSONAL CARE AGENCY LICENSING RULE
Section 144-129-8 - SERVICES

A. SERVICE PLAN DEVELOPMENT AND IMPLEMENTATION.

1. A written service plan must be developed, in coordination with the client and/or legal representative, prior to any services being provided.

2. The service plan must identify the following:
a. The date the service plan was developed;

b. Who participated in the development of the plan;

c. The services that are to be provided;

d. The frequency at which each service is to be delivered; and

e. The client's and/or legal representative's signature attesting to their involvement and agreement with the plan.

3. The service plan must be updated whenever there is a significant change in client function or status. At the time of any update, the client and/or legal representative will sign attesting to their involvement and agreement with the updated service plan.

4. A review of the service plan, with the client and/or legal representative, must be conducted at least annually. At the time of any update, the client and/or legal representative must sign attesting to their involvement and agreement with the updated service plan.

5. The agency must implement the service plan as written.

B. DOCUMENTATION OF SERVICES PROVIDED.

1. At each visit, the employee must document what services were provided to the client.

2. Documentation must include the signature of the employee and date of service(s) provided.

3. If the client expresses any grievances regarding his/her services, the employee must document these concerns in the client's record and report the concerns in accordance with the agency policy.
