09/01/07 A change of administrator must be reported to the Department in writing at least thirty (30) days prior to the change taking effect. In emergency situations, the Department must be notified immediately.
The name of the new administrator, his/her qualifications, facility address, home and office telephone number and e-mail address must be included. A new license shall be issued within thirty (30) days upon payment of a $25.00 fee, pending approval of the new administrator.
