Section 137-23-1.04 - Camp Trip Leader Safety Course Application Form

An application form for the Camp Trip Leader safety course is to be filled out by each candidate, the candidates Camp/organization Director, and Course Instructor where indicated and/or applicable. The following information is required:
A. Name and legal residence of applicant.
B. Date of birth of applicant.
C. Name and address of applicants sponsoring Camp.
D. Signature of applicants sponsoring Camp Director. This signature is given attesting the candidate has all required knowledges and prerequisites.
E. Name and address of course location.
F. Signature of Course Instructor(s). This signature is given attesting the candidates having successfully completed the course and examination.
G. A space to indicate applicants final score after completion of course.
