Section 502-3-A-4 - Application

A. Generally
A licensed acupuncturist shall apply for certification to practice the formulation and dispensing of custom-made Chinese herbal formulations on forms provided by the board. The statements from licensees, colleagues or patients required by Section 1(D) of this chapter shall be made on forms provided by the board. An applicant who is not licensed as a Maine acupuncturist at time of application must also simultaneously apply for licensure as an acupuncturist pursuant to Chapter 3 of the board's rules. The applicant shall submit the certification fee set forth in Chapter 10, Section 4(12) of the rules of the Office of Licensing and Registration, entitled "Establishment of License Fees," with the application and shall provide such additional information as the board may require.
B. Experienced Maine Herbal Practitioners
A Maine acupuncturist applying for transitional certification pursuant to Section 2 of this chapter on the basis of the practice of custom-made Chinese herbal formulation prior to July 1, 2004 shall submit the statements from licensees, colleagues or patients required by Section 2 of this chapter on forms provided by the board.
C. Herbal Practitioners Licensed in Another State
An acupuncturist applying for certification pursuant to Section 3 of this chapter on the basis of a license issued by another state shall:
