Section 392-4-19-1 - General Requirements for Prescription Drug Orders

1. Required Information
Prescription drug orders shall contain, at a minimum, the following information:
A. Date of
issuance by practitioner;
B. Name
and address of the patient [or patient location if an institution];
C. Name and address of the practitioner [if
not a staff physician at an institution];
D. DEA number of practitioner [in the case of
controlled substances];
E. Name,
strength, dosage form and quantity [or stop date, and route of administration]
of drug prescribed;
F. Refills
authorized; and
G. Directions for
use by patient.
2. Verification
The pharmacist who receives a prescription drug order shall record the order and verify the identity of the practitioner and, if applicable, the identity and authority of the practitioner's agent.
