Section VII-1513 - Comprehensive Assessment

A. Purpose
1. To make a determination of the independent
living needs of the individual with a disability.
2. To make a determination of the objectives,
nature, and scope of independent living services required for development of
the Independent Living Plan (ILP) of an eligible individual.
B. Scope. To the extent additional data is necessary, LRS shall conduct a comprehensive assessment to determine the unique strengths, resources, priorities, concerns, abilities, capabilities, interests, and informed choice, of the eligible individual.
C. Additional Considerations
1. The comprehensive assessment is limited to
information necessary to identify the independent living needs of the eligible
individual and to develop the Independent Living Plan (ILP).
2. LRS will use as a primary source of
information, to the maximum extent possible and appropriate, existing
information obtained for the purpose of determining eligibility.
3. LRS will use, to the maximum extent
possible and appropriate, information provided by the individual and/or the
individual's family.
