Chapter 10 - Deferred Retirement Option Plan (DROP)
Section XXV-1001 - Application Process

A. An application for DROP may be made when a member is eligible for retirement.
B. The DROP application shall include:
1. a form submitted by the
member's employer providing data on the member's salary for the highest 60
consecutive or joined months of earnings;
2. a written acknowledgement by the member of
the number of months in which they will participate in DROP;
3. a copy of the member's birth certificate
and Social Security card;
4. a copy
of the beneficiary's birth certificate and Social Security card;
5. a designation of a beneficiary to receive
the DROP fund balance if the member dies while participating in DROP;
6. a spousal consent form as to the
retirement benefit if the member is legally married and not selecting a benefit
which provides at least 50 percent to the spouse;
7. a spousal consent form as to the DROP
funds if the member is legally married and not leaving at least 50 percent of
their DROP fund balance to their spouse;
8. a copy of the spouse's death certificate
if the member is widowed;
9. a
certified copy of the divorce judgment if the member is divorced; and
10. for elected officials, a certificate of
elected service.
