Section III-2107 - Application for Hang Tags (Applicant is Unable to Visit a Field Office)

A. First time applicants:
1. a currently dated medical
examiner's certification form or, in the case of a disabled veteran, proof of a
special disabled veteran license plate, a decision letter or an affidavit from
the Office of Veteran Affairs and a separate medical examiner's certification
which indicates the applicant is unable appear in person at the Office of Motor
Vehicles;
2. a color photograph of
the applicant.
B. Renewal applicants:
1. if the applicant has a
permanent status-mobility ID and expired hang tag;
2. if the applicant has a temporary status-a
currently dated examiner's certification of mobility impairment form (DPSMV
1966) or, in the case of a disabled veteran, a decision letter or an affidavit
from the Office of Veteran Affairs and a separate medical examiner's statement
indicating the applicant is unable to appear in person at the Office of Motor
Vehicles;
3. a color photograph of
the applicant.
