A. In order to participate in the Medicaid Program, a provider must meet all of the following requirements.
1. The provider must meet all the
requirements for licensure as established by state laws and rules promulgated
by the Department of Health (LDH) or have a current, valid license or
certification from the appropriate governing board for that profession.
2. The provider must agree to
comply with all the terms and conditions for Medicaid enrollment as contained
in:
a. the provider enrollment
packet;
b. the Medical Assistance
Program Integrity Law (MAPIL),
R.S.
46:437.1-440.3;
c. the provider agreement;
d. the standards for participation contained
in the Children's Choice and Case Management Services provider manuals;
and
e. all other applicable federal
and state laws, regulations and policies.
3. All services must be appropriately
documented in the provider's records.
