A. General Requirements. The ICF/MR shall maintain medical records which include clinical, medical, and psychosocial information on each client.
B. Components of Medical Records. Each client's record shall consist of a current active medical section and the ICF/MR's medical files or folders.
1. Active
Medical Section. The active medical section shall contain the following
information:
a. at least six months of
current pertinent information relating to the active ongoing medical
care;
b. physician certification of
the clients' need for admission to the ICF/MR;
c. physician recertification that the client
continues to require the services of the ICF/MR;
d. nurses quarterly physical assessment. See
§31101, Client Health and Habilitative Services;
e. quarterly, the pharmacy consultant must
review the drug regimen of each client;
f. certification that each IHP has been
periodically reviewed and revised.
2. Medical Files. As the active medical
section becomes bulky, the outdated information shall be removed and filed in
the ICF/MR's medical files.
