A. The Infant Intervention Program shall provide OMR/DD with a monthly report which indicates the following:
1. days in attendance by each
infant in center-based services;
2.
units of service provided to each infant and parent(s) in home training
services;
3. infants admitted
during the month;
4. infants
terminated during the month and to where terminated.
