A. The facility shall establish and have written grievance procedures to include, but are not limited to:
1. a formal process to
present grievances;
2. a formal
appeals process for grievances; and
3. a process to respond to client requests
and/or client grievances in a timely manner, and the time frames in which the
facility shall respond.
