A. LDH shall be notified, in writing, of any of the following within five working days following the occurrence:
1. address/location
(an inpatient hospice facility shall notify and receive approval by LDH prior
to a change of address/location)-fee required;
2. agency name - fee required;
3. phone number;
4. hours of operation/24 hour contact
procedure;
5. ownership
(Controlling) - fee required;
6.
change in address of any branch office-fee required;
7. administrator (completed key personnel
change form, obtained from LDH required);
8. director of nursing (completed key
personnel change form required); or
9. cessation of business in accordance with
the requirements of §8243
B. Change of Ownership. A representative of the buyer shall request approval for a change of ownership prior to the sale.
1. Submit a written notice to LDH for a
change of ownership. Change of ownership (CHOW) packets may be obtained from
LDH. If the hospice had less than two active patients at the time of the most
recent survey, and less than twenty new patients admitted since the last annual
survey, the department may have issued a provisional license. Only an agency
with a full license shall be approved to undergo a change of
ownership.
2. Submit the following
documents for a CHOW:
a. a new license
application and the current licensing fee. The purchaser of the agency shall
meet all criteria required for initial licensure for hospice in accordance with
the provisions of §8203;
b. any
changes in the name and or address of the agency;
c. any changes in administrative
personnel;
d. disclosure of
ownership forms; and
e. a copy of
the bill of sale and articles of incorporation.
3. Repealed.
