A. The Department of Health and Hospitals shall be notified, in writing, of any of the following within five working days of the occurrence:
1. change in physical address. (An agency
must notify and receive approval from DHH prior to a change of physical
address);
2. change of agency
name;
3. change of phone
number;
4. change of hours of
operation/24 hour contact procedure;
5. change of ownership
(controlling);
6. change in address
or phone number of any branch office;
7. change of administrator (completed Key
Personnel Change Form, obtained from DHH, is required); and
8. change of director of nursing (completed
Key Personnel Change Form required); or
9. cessation of business.
B. Change of Ownership
1. Change of Ownership (CHOW) packets may be
obtained from DHH. Only an agency with a full license shall be approved to
undergo a change of ownership. A CRCC license is not transferable from one
entity or owner to another.
2. The
following must be submitted within five working days after the act of sale:
a. a new license application and the current
licensing fee. The purchaser of the agency must meet all criteria required for
initial licensure for CRCC;
b. any
changes in the name and/or address of the CRCC;
c. any changes in administrative personnel
(DON, administrator, medical director);
d. disclosure of ownership forms;
e. a copy of the Bill of Sale and Articles of
Incorporation.
