A. Any remittance submitted to the department in payment of a required fee must be in the form/manner specified by the department.
B. Fee amounts shall be determined by the department in accordance with R.S. 40:1135.4 et seq.
C. Fees paid to the department are not refundable.
D. A fee is required to be submitted with:
1. an initial
application;
2. a renewal
application;
3. a change of
controlling ownership;
4. a change
of name or physical address; and
5.
each application for a permit to add a vehicle to the service.
