A. Providers shall ensure that each family receiving services is assigned a service coordinator.
B. The service coordinator shall perform the following tasks:
1. prepare a family study,
based on a home visit interview with the client, in order to ascertain what
appropriate family support services may be provided;
2. visit each client at least
quarterly;
3. maintain
documentation of all significant contacts; and
4. review and evaluate, at least every six
months, the care, support and treatment each client is receiving.
