A. A provider shall have a written program description describing:
1. the overall philosophy of the
provider;
2. the long-term and
short-term goals of the provider;
3. the types of clients to be served by the
provider;
4. the services provided
directly by the provider;
5.
services available to the client from resources external to the
provider;
6. a schedule for any
fees for services;
7. the
provider's approach to service planning.
