A. When there are substantiated charges against the nurse aide, either through oral or written evidence, the department shall notify the individual(s) implicated in the investigation of the following information by certified mail:
1. the nature of the violation(s) and the
date and time of each occurrence;
2. the department's intent to report the
violation(s) to the Nurse Aide Registry; and
3. the right to request an informal dispute
resolution and/or the right to an administrative hearing.
