A. A physician engaged in collaborative drug therapy management shall:
1. annually report,
as a condition to the renewal of that physician's license, whether or not and
the extent to which the physician is engaged in collaborative drug therapy
management and such other information as the board may request; and
2. within 15 days of the occurrence or
discovery notify the board in writing of complications or errors that are, in
the physician's opinion, directly related to drug therapy mismanagement;
and
3. comply with reasonable
requests by the board for personal appearances and/or information relative to
the functions, activities and performance of a physician or pharmacist engaged
in collaborative drug therapy management.
