A. No registrant shall dispense any medication, other than a bona fide medication sample, unless the bottle, package, or other container for such medication bear a securely-affixed indelible, legible, typewritten, or printed label including:
1. the name and address of the
registrant;
2. the name of the
patient to whom or for whom dispensed;
3. the generic chemical or trade name,
quantity or amount, dosage form, and strength of the medication
dispensed;
4. the date of
dispensation; and
5. appropriate
directions for self-administration, ingestion, insertion, application, or
injection by the patient.
