A. Third -party logistics provider shall report to the appropriate federal official on an annual basis on a schedule determined by the appropriate federal official:
1. the state in which the third-party
logistics provider facility is licensed and the appropriate state license
number issued by the state to the third-party logistics provider; and
2. the name and address of the third-party
logistics provider facility and all trade names under which the third -party
logistics provider facility conducts business.
