A. A complaint may be initiated by any person, corporation, association, public officer, or by the board on its own initiative.
1. Anonymous sources of complaint
against individuals or institutions shall not be recognized as the sole basis
for the issuance by the board of a full notice as provided for under
§707. A.3 However, the
executive director and his/her designee or staff may informally investigate
anonymous allegations or complaints received by the board.
2. If the information provided to the board
by the complainant is insufficient, the board may request further information
by either written correspondence or through an informal proceeding.
B. All complaints received shall be assigned a sequentially ordered complaint code number which shall be utilized in all official references.
C. The board shall act upon all complaints received without unreasonable delay.
D. The board shall take one of the following actions on a complaint:
1. no
action;
2. informal
proceeding/consent order;
3. formal
hearing.
