A. Plan shall in its explanation of benefits provided to covered persons, include the following language. "If you are covered by more than one health benefit plan, you should file all your claims with each plan." Additionally, notice to obtain a copy of Appendix C, as provided for in LAC 37:XIII.305.G, shall be added as part of the coordination of benefit section or provision found in an insurance contract or subscriber agreement. Appendix C will also be available on the Louisiana Department of Insurances website.
