Section XI-1105 - Petitions, Complaints or Orders

A. The applicant desiring, or required by law, to institute a hearing shall prepare and file with the commissioner a petition, complaint or order to show cause setting forth:
1. the name and address of each
respondent;
2. a statement, in
ordinary and concise language, of the facts upon which the petition, complaint
or order to show cause is based, together with supporting evidentiary material
including, whenever applicable, particular reference to the statute or
statutes, or rules, regulations, and orders that the applicant alleges have
been violated.
