Section 28-35-281 - Utilization logs

(a) Each licensee or registrant shall maintain a log for each source of radiation, which shall contain the following information:
(1)
The make and model number, or a detailed description, of the source of
radiation or storage container to which the log pertains;
(2) the name and signature of the
radiographer to whom the source or container is assigned;
(3) the plant or site where the source or
container is used;
(4) the date or
dates when the source or container is used; and
(5) the voltage, current, and exposure time
for each radiographic exposure made with a radiation machine.
(b) Each licensee or registrant shall retain the logs required by this regulation for three years.
